A convenient alternative to in-person primary care, Centivo Care provides a friendly, accessible care team who gets to know you and your needs.
See your Centivo Care team for wellness exams, when you’re sick, for chronic condition management and mental health. They’ll even coordinate any in-person care needs with nearby, in-network providers.
Members 18 an older will automatically become a patient of Centivo Care.
Search for a doctor or facility
The Centivo Basic PPO Plan available in Ohio uses the Cigna PPO Network.
Click “Find a doctor or facility” to search the network. When asked to login/register, choose “Continue as guest”, and when asked to select a plan, choose “PPO”. Note: this site is provided and managed by Cigna and may include providers not available with your plan.
